What is ACL reconstruction?

Anterior cruciate ligament or ACL reconstruction is a surgical procedure to repair a torn ACL, which is a ligament that helps connect thigh bone (femur) to shinbone (tibia). ACL injuries commonly occur during sports that involve sudden stops or changes in direction, jumping and landing.

Pivoting with your foot firmly planted
Landing awkwardly from a jump
Stopping suddenly
Receiving a direct blow to the knee
Suddenly slowing down and changing direction

A loud pop in the knee
Severe pain
Rapid swelling
Loss of range of motion

The doctor physically examines by placing hands on different parts of patient’s knee gently and checks the swelling and if the knee is functioning properly. If any of your bones move in a way that isn’t normal that could be a sign that your ACL is damaged. The doctor may also recommend tests such as:

X-ray. Soft tissues like the ACL don’t appear on X-rays, but your doctor may want to rule out broken bones.

MRI or ultrasound. These exams can show both soft tissue and bone. If you have a damaged ACL, it should appear on the images.

Arthroscopy. During the examination, an orthopedic surgeon makes a small cut in your skin. He inserts a pencil-sized tool that contains a lighting system and lens (arthroscope) into the joint. The camera projects an image of the joint onto a TV screen. Your doctor can see what type of injury you have.

In ACL reconstruction surgery, the surgeon removes the injured ligament and replaces it with a segment of tendon tissue from another part of the knee or body to help a new ligament grow in its place. The tendon tissue works similar to that of a ligament which connects the muscle to bone. Sockets are drilled into the thigh and knee bone to hold the new graft.

For the next few days, complete bed rest is required with the leg kept elevated and ice is applied to keep the swelling and pain low. Putting weight on the treated leg is to be avoided and crutches and knee braces are to be used. With physiotherapy, people who have surgery can often play sports again within 12 months.
