What is PCL reconstruction?

Posterior cruciate ligament or PCL reconstruction is a surgical procedure to repair a torn PCL, which is a ligament that connects your upper leg to your lower leg. Even though your PCL is stronger and larger than your anterior cruciate ligament (ACL), it can still be injured.

The doctor will press on your knee to feel for injury, looseness or fluid in the joint from bleeding, then compare your injured leg with the healthy one to look for any sagging or abnormal movement in the knee or shinbone. The doctor may recommend tests such as:

X-ray. While an X-ray can't detect ligament damage, it can reveal bone fractures.

MRI scan. An MRI scan can clearly show a posterior cruciate ligament tear and determine if other knee ligaments or meniscus or cartilage also are injured.

Arthroscopy. The doctor might use a surgical technique called arthroscopy to look inside your knee joint, a tiny video camera is inserted into your knee joint through a small incision.

Surgery is carried out with the help of an arthroscope using a few small incisions, the donor tendon is harvested from the patellar tendon or the semitendinosus and gracilis tendon, first the remains of native PCL is removed then the soft tissue around the thigh bone (femur) is debrided to assist the insertion of the graft. The tendon allograft is inserted in the tunnel and fixed with a screw. The graft is then fixed to the shinbone (tibia), with the help of staples. The incision is closed with sutures and covered with sterile dressings.

Physiotherapy is recommended for best results and recovery to normal routine.
